Kenner guides patients to new model of care program

Do you know which Patient Centered Medical Home team you are on?

Do you know who your primary care manager is?

At Kenner Army Health Clinic, we have assigned all of our medical beneficiaries to a smaller unit of care within the clinic due to our adoption of the Patient Centered Medical Home model of care.

This is an innovative, team-based approach to providing health care. PCMH is a transition from an older model of episodic care (random appointments with different providers) to a partnership between the patient and his or her health care team. This promotes more communication between the patient and the health care team in between visits and more consistent contact with the same medical provider and nursing staff.

With PCMH, patients receive timely reminders when tests are due or for follow-up appointments. The health care team works with its patients to help them navigate the system and ensure that all concerns are addressed.

There are six health care teams:

Family Medicine Clinic has the James Team and the ?"York Team.

Active Duty Clinic has the Honor Team and the Valor Team.

Pediatrics Clinic has the Horse Team"and the Zebra Team.

The adoption of PCMH has effectively increased the Team and Primary Care Manager continuity of patient encounters from approximately 30-40 percent to 60-80 percent over the past 18 months, resulting in an increase in the team?'s ability to get to know their patients, provide better care because of this familiarity, and the ability to anticipate and provide better preventive medicine services and education.

Increased communication and access to care with your team is a key initiative that we are proud to say is improving by leaps and bounds.

Kenner has several ways patients may contact their health care team and provider. In addition to our call center (1-866-LEE-KAHC), TRICARE Online and after-hours provider access line (804-734-9000); we now offer a secure messaging system called ?"Relay Health" in which you are able to enroll at your clinic.

Relay Health is a secure system that has strict privacy practices that protect your health care information. A personal face-to face verification of your identity is required in order for you to sign up. Once registration and validation of your account is complete, you may start taking advantage of the system that allows you to send and receive secure messages. These would include requests for medication refills, follow ups on laboratory results, requests for appointments or a call back to you about any concerns.

The NCQA certification ensures that patients enrolled at Kenner enjoy a consistent health care experience by enhancing access to care and patients?' continuity with their provider teams.
