Thank you for your continued patience as we expect Army guidance concerning the Executive Order for federal employees to be fully vaccinated against COVID-19. We have recently received guidance to begin the process of documenting DoD Civilian teammates in order to meet the 22 November deadline.
All DoD Civilians are required to complete a DD Form 3175 attesting to their vaccination status and provide proof of vaccination to their supervisor. Employees may complete this form online via milConnect at https://milconnect.dmdc.osd.mil or provide a hard copy to their supervisor. Proof of vaccination in the form of an immunization record from a health care provider or pharmacy, the COVID-19 vaccination record card, medical records documenting the vaccination, or any other official document containing the required information must be submitted to your supervisor. Each proof of vaccination must contain: the type of vaccine administered, number of doses received, date(s) of administration, and name of health care professional or clinic site who administered the vaccine.
Supervisors will then complete Section B of the DD3175 confirming receipt of vaccination proof and may do so on milConnect once supervisor functionality becomes available online. Supervisors are required to report status data through their organizational channels on a weekly basis. As a reminder, DoD Civilians must be fully vaccinated by 22 November 2021 regardless of telework or remote work status.
I understand the uncertainty surrounding this process, and I appreciate your cooperation and patience as we wait for further guidance from HQDA, including guidance on processing refusals and exemptions. At this time, supervisors are not authorized to take any disciplinary action against refusals nor process or approve any requested exemptions. Additionally, Civilians who are not fully vaccinated are not required to show proof of negative test to enter DoD facilities until at least 22 November 2021. Further guidance on test requirements is forthcoming.
