USAHC-SB beneficiaries; it's never too early to schedule your child's school physical

With school starting in just under one month, parents are beginning to prepare their children. School supplies and new clothes are all at the top of parent's list of things to buy for their kids.

But what about their school or sports physical? Is your child current?

"If your child gets an annual physical, that can also be counted as the school and sports physical" says Dr. Christopher McEvoy, Medical Director of the Pediatrics Clinic at U.S. Army Health Clinic-Schofield Barracks (USAHC-SB). "The goal is a single checkup, with everything done at that visit, regardless of when during the year the visit takes place."

Children need a single school physical upon entering the school system. Sports and CYS physicals need to be updated annually.

If your child has had a physical within the past nine months, your care team can often complete the physical forms without the need for an additional physical.

If you're wondering how to do this, the process is fairly simple. You can request an appointment or complete forms online by messaging your Primary Care Team via the Secure Messaging Service (SMS) at www.relayhealth.com.

Here in Hawaii, you can also call the Appointment Line, at (808) 433-2778 and press 1,1 at the voice prompt.

If you are a U.S. Army Health Clinic-Schofield Barracks patient and can't get an appointment that meets your needs, or if you simply need the forms completed, call (808) 433-2778, then press 3,1,1,1 for the Family Medicine clinic, or 3,1,1,2 for the Pediatrics Clinic.

To make the process run more smoothly, consider filling out the forms your child needs in advance.

The School Physical Form 14 and School Medication Forms can be accessed on the Hawaii Department of Education website at http://www.hawaiipublicschools.org.

CYS forms can be downloaded at http://www.himwr.com/program-registration.

Bring the completed forms, your child's immunization records and any paper health records to the appointment.
