FORT BENNING, Ga., (June 24, 2015) -- Martin Army Community Hospital will offer school and sports physicals for Tricare Prime beneficiaries enrolled in Pediatric Medical Home and Family Medical Home on select dates this summer.
The all-inclusive service will cover medical services a child will need to enroll in school or sports, said Maj. Ila Moffitt, Pediatric Clinic medical director.
Appointments will be available on the following dates:
8 a.m. to noon June 27, July 18 and Aug. 8
9 a.m. to 4 p.m. July 31
"With this one-stop-shop, we will provide efficient and timely access to school and sports physicals for the children of Martin Army Community Hospital, Moffitt said.
A copy of immunization records and any required forms from school and sports programs should be brought to the appointment.
Parents of children with chronic conditions such as asthma, heart issues, ADHD, ADD, depression or other special considerations should schedule a school and sports physical appointment with their Primary Care manager.
Patients enrolled at North Columbus Medical Home can schedule school and sports physical appointments with their providers.
