KINDERGARTEN REGISTRATION FOR FALL 2015

LOCATION:
Registration will be held at all Tooele County elementary schools on February 26th and 27th.
ELIGIBILITY:
To be eligible to enter kindergarten a child must be at least five (5) years of age on or before
September 1, 2015.
DOCUMENTATION REQUIRED BEFORE A CHILD CAN OFFICIALLY BEGIN SCHOOL:
• Official State Birth Certificate - A copy of a state birth certificate for a child born within Utah may be obtained from the Tooele County Health Department, 151 North Main, Tooele. A copy may also be obtained from the Utah State Division of Health, 554 South 300 East, Salt Lake City. Birth certificates for a child born outside of Utah should be obtained from the State Division of Health in the state the child was born.
• Immunization Record - The following are required by law for a child entering school (see attached photo)
• Vision Screening -- A child under seven (7) years of age entering school for the first time in the state of Utah must present a certificate signed by a licensed physician, optometrist, or other licensed health professional, stating that the child has received vision screening (Utah Code-Title 53A-11-203).
RECOMMENDATIONS:
The following are highly recommended by the Tooele County School District before registering:
1. Health Exam - make an appointment with your physician or health clinic
2. Dental Exam
3. 4 doses of Haemophilus Influenza Type B (Hib) - dosing scheduled based on student's current age
KINDERGARTEN ASSESSMENT:
The kindergarten prescreening will be scheduled sometime prior to the end of this school year. It is important that you have your child registered prior in order to receive information from your school regarding assessment times. Students will be assigned a kindergarten teacher by the principal with the assistance of the kindergarten staff after all prescreening is completed.
KINDERGARTEN ORIENTATION:
A kindergarten orientation will be held at local schools in August where general kindergarten information will be shared.
